    VISN 21 One View is partnering with National CRH for innovation diffusion. A diffusion plan will be developed during the Diffusion of Excellence Base Camp.
  • Facilitated Replication (Feb - Oct 2024)
    VHA Diffusion of Excellence Implementation Leads and Coordinators will work with the VISN 21 One View and National CRH teams to replicate the Promising Practices using the implementation plan developed at Base Camp.
  • National Stakeholder Engagement (June - Aug 2024)
    The National CRH team will coordinate with Implementation Leads and Diffusion Specialists to develop a National Stakeholder Strategy and schedule information sessions.

I am a VISN 21 VA Tele-Emergency _ Urgent Care NP and I have found such value in this dashboard. In my position, I toggle between 6 different CPRSs. This dashboard helps, at a glance, see my alerts at each station. I can then triage which I need to view first and plan responding to the Veteran (examples labs and imaging results vs response to a consult). I feel that has increased my efficiency and safety of practice. Thank you and very happy this is innovation has made the list!

Origin story

The VISN 21 Clinical Resource Hub (CRH) is a national leader in providing telehealth. It has observed significant growth in the number of services being offered and in the number of providers delivering care at numerous sites. The VA electronic health record (VistA) generates a view alert when a clinical activity requires attention. As monitoring view alert ... The VISN 21 Clinical Resource Hub (CRH) is a national leader in providing telehealth. It has observed significant growth in the number of services being offered and in the number of providers delivering care at numerous sites. The VA electronic health record (VistA) generates a view alert when a clinical activity requires attention. As monitoring view alerts requires that CRH providers to access individual facility EHRs, it becomes challenging to keep up with a large number of Veteran view alerts at multiple facilities. These barriers may result in delays with viewing vital alerts (e.g., laboratory results, imaging results, etc.) and potentially creates unintended consequences of increased turnaround times in relation to communicating results to Veterans and taking action where required. Our VISN 21 Data Analytics Team developed an interactive dashboard that uses near real time data to display pending view alerts from facilities across the VISN. The report allows providers to triage and prioritize their workload. This tool meets CRH provider timeliness demands and ensures the highest levels of patient safety and high reliability by simplifying the process of tracking view alerts.
