About Age-Friendly Health Systems

Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).

In March 2020, GEC set the aim for VHA to become the largest integrated health care system in the U.S. to be recognized by the Institute for Healthcare Improvement (IHI) as Age-Friendly. Since then, more than 100 VA medical centers across the country have joined the movement as official Age-Friendly Health Systems Participants.

    Executive Director

    Office of Geriatrics & Extended Care 

    Dr. Hartronft assumed the role of Executive Director, Office of Geriatrics and Extended Care (GEC) effective August 18, 2019. As Executive Director, his responsibilities cover the care of Veterans with complex care needs of all ages. GEC programs range in health care settings to include, but not limited to, acute care, home care, extended care, purchased home, hospice and palliative care, community, and facility-based community care. VA GEC programs  accounted for over $10 billion in expenditures for FY20. Prior to August 2019, Dr. Hartronft served in senior VA field positions at three different complexity 1a VA Medical Centers since 2006 including Chief of Staff at VA Greater Los Angeles Health Care System, Deputy Chief of Staff VA Puget Sound Health Care System, and Associate Chief of Staff at South Texas Health Care System in San Antonio.

    Age-Friendly Program Analyst

    Office of Geriatrics and Extended Care

    Ines is a Program Analyst for Age-Friendly Health Systems with the VA Office of Geriatrics and Extended Care. With a background in administration and clinical operations, she previously served as the business manager for Rehab & Extended Care at the VA Hudson Valley Health Care System. An Army Veteran, Ines completed a year-long deployment to Afghanistan in support of Operation Iraqi Freedom and Operation New Dawn.

    Age-Friendly Program Analyst

     Office of Geriatrics and Extended Care

    Kayla is a Program Analyst for Age-Friendly Health Systems with the VA Office of Geriatrics and Extended Care. With 10 years of social work experience in geriatrics and end-of-life care at the William S. Middleton VA Hospital in Madison, WI, she has also worked in Systems Redesign, Caregiver Support, Oncology, and inpatient social work. Kayla helped develop the national Goals of Care Conversations Train-the-Trainer program with the National Center on Ethics in Health Care.

    VA Age-Friendly Health Systems Steering Committee, Incoming Chair

    Dr. Jindal, a Geriatric Medicine physician at Cincinnati VA Medical Center, serves as Transition of Care Director and Associate Professor of Clinical Medicine at University of Cincinnati College of Medicine. She also consults for VA National Office of Geriatrics & Extended Care, focusing on Home and Community Based Programs. With a residency in internal medicine from University of Cincinnati, she pursued Geriatric Medicine fellowship at Boston University Chobanian & Avedisian School of Medicine and advanced fellowship at VA Boston Healthcare System. Her work spans hospitalist and educator roles across Veterans Health Administration, emphasizing quality care for older adults, care innovations, and humanism in medicine. She focuses on Age-Friendly Health Systems Initiative implementation within VA system and various clinical settings.

    VA Boston Healthcare System

    Andrea Wershof Schwartz, MD MPH AGSF is Associate Director for Clinical Innovation at the New England Geriatric Research Education and Clinical Center and a geriatrician at VA Boston Healthcare System. She is also Associate Professor of Medicine at Harvard Medical School, co-directing the Aging and End of Life Theme for medical students, and serving as Associate Program Director for the Harvard Multicampus Fellowship. Additionally, she is Associate Professor of Epidemiology at Harvard T.H. Chan School of Public Health and faculty in the Institute for Healthcare Improvement’s Age-Friendly Health System Initiative. Dr. Schwartz received the American Geriatrics Society’s Outstanding Junior Clinical Award and mentors trainees in Geriatrics and medical education.

    Kristen Sorocco, PhD

    VA Oklahoma City Healthcare System

    Kristen H. Sorocco received her Ph.D. in clinical psychology and a certificate in Gerontology in 2001 from Oklahoma State University.  She returned to Oklahoma after completing her clinical internship in California at the Palo-Alto Veteran’s Administration Health Care System.  During her internship year she received specialized training in geropsychology. She completed her postdoctoral training in Biological Psychology through the Department of Psychiatry and Behavioral Sciences at the University of Oklahoma Health Sciences Center.  Currently, she is the Psychology Training Director and Assistant Chief at the OKC Veterans Affairs Medical Center, Oklahoma City, OK. and an Associate Professor within the Department of Internal Medicine, Donald W. Reynolds Section of Geriatrics and Palliative Medicine at the University of Oklahoma Health Sciences Center. Dr. Sorocco is currently serving as the Chair for VA Age Friendly Health Systems Steering Committee. The OKC VA HCS geriatric inpatient mental health program achieved Committed to Care Excellence designation for Age Friendly Health Care System in 2022.

Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA). The goal is to build a social movement so that all care with older adults is Age-Friendly.

IHI recognizes clinical care settings that are working towards reliable practice of evidence-based interventions for all older adults in their care known as the 4Ms (4Ms: What Matters, Medications, Mentation, Mobility). As of August 2021, there are over 2,400 hospitals and health practices that have earned level 1 (Participant) recognition and 615 that have earned level 2 (Committed to Care Excellence) recognition in the Age-Friendly Health Systems movement.

  •  Level 1 (Participant) teams have successfully developed a plan to implement the 4Ms.​
  •  Level 2 (Committed to Care Excellence) teams have three months of verified data to demonstrate early impact of using the 4Ms.

There is no cost associated with Age-Friendly Health Systems recognition. Thanks to generous support from The John A. Hartford Foundation, IHI offers recognition at no cost to health systems. ​

VA is working to become the largest integrated Age-Friendly Health System in the U.S. As of April 2024, over 350 VA teams have earned recognition from IHI for their commitment to Age-Friendly care for older Veterans.

When practiced reliably as a set, the 4Ms represent a broad culture shift to focus and align care based on each Veteran’s unique care preferences and health outcome goals. By implementing the 4Ms, VA is taking a high-reliability approach to continuously improve care and achieve zero harm.
