    Identify CPhT to support PACT population management
    Outline current process for PACT CPP population management Veteran recruitment and scheduling
    Review PACT CPP established grids for face-to-face/telephone/VVC clinic capabilities based on PACT CPP Direct Patient Care time
    Encourage leadership opportunity among frontline PACT CPP
  • Week 1
    Identify and evaluate opportunities for CMM based on facility needs
  • Week 2
    Choose 1-2 PACT CPP as lead(s) for CPhT
    Educate PACT CPP lead(s) on overall process and CMM activities targeted
  • Week 3
    Lead introductory meeting between PACT CPP lead(s) and CPhT to review training plan
  • Week 4
    Review PACT CPhT competency
  • Weeks 5 - 6
    Create non-count CPhT phone clinic and note titles
    Create CPhT disease-state specific templates
  • Week 7
    CPP lead(s) review dashboards and specific patient data with CPhT and determine appropriateness of patient for CPP management

    CPhT completes templated notes in patient record
    CPhT completes RTCs for appropriate CPP clinic
    Set target number of patients recruited for each CPP clinic

Risk Mitigation
Resistance to change from CPPs Engage PACT CPPs to be leads in process change. Ensure open discussion regarding the responsibilities of PACT CPPs and the CPhT's direct supervisor.
CPhT not meeting measured workload Consider additional training and review CPhT's population management recruitment process. Evaluate measurable and achievable workload for CPhT. Ensure CPhT aware of recruitment goals and expected productivity.
